We are here to help you enjoy your life. To initiate therapy services, simply call your child’s pediatrician and have them fax the referral to our office at (704) 775-4466 to process and the office staff will reach out to you to schedule the initial evaluation appointment.

Before Your First Visit

• Make a list of questions and concerns you would like to discuss with your therapist.
• List signs and symptoms that can be of importance to the therapist.
• Have in writing key past medical history, previous treatments, current medications, and other important information.
• Please call our office to verify your insurance benefits.
• Arrive 15 minutes early for your first appointment.
• Bring with you a copy of your insurance card and a photo ID.

What to Expect Prior to Your First Visit:

Once the office staff receives and processes the referral, they will call you to go over insurance benefits, schedule the evaluation(s) appointment(s) and email you the intake paperwork and the invitation to the portal.

What to Expect on Your First Visit

• If your intake paperwork has not been completed by your appointment date, you will need to complete at this time.
• If an initial payment needs to be made, the office staff will go over the payment and any additional information needed, as well as go over any questions you may have at that time.
• You will meet your child’s therapist for the initial evaluation.
• During the evaluation, your child’s therapist will ask questions about your child’s medical history, birth history, questions pertaining to the reason for the referral and your concerns for your child.
• Our purpose is to help you navigate through your child’s therapy journey.
• Once the evaluation process is complete, the therapist will inform you of their findings, complete the documentation process, the office staff will submit for authorization from the insurance company, and the therapist will then contact you to schedule weekly therapy session(s).
